The Communication Forms

All of our communications will be conducted by using the forms below. To get started, use the Apply button: this form contains the screening questions and the consent acknowledgments. If you are accepted, your enrollment confirmation email carries your personal code. That code will open your Intake form. Once you complete and submit that form, you will receive a welcome email and how to get started. The FAQ on the beta page is where the study’s answers live for the whole trial. When you have finished, the same code opens the Exit form — the core data of the trial. Name and DOI correction is used only if the study emails you that it could not find your Zenodo deposit; before you resend, go to Zenodo and make sure you copied the right number. The last form is Withdraw. If you want out, please use it rather than just stopping — someone who just stops, without a word, is logged as incomplete, and knowing the difference helps us improve the method. If your email program doesn’t open when you press the button, use the Copy buttons that appear and paste the three pieces into a new email to the address shown.

APPLY
Your code (it looks like ET-0906-03). The form can’t check it; the study can. Every form you send is answered: confirmed, or with the notice: “NOTICE: your ETR code was not accepted. Please check your code and try again. The form must also come from the email address your code was sent to.” Nobody at the study answers or guides participants during the trial.

Email is a postcard, not a locked safe. No privacy is promised. Never send your passphrase, work, files, archive, evidence, attachments, pasted work, or screenshots.

1. Apply

One form, one send, no code needed. If you take part, your name and your work’s DOI will be published in the report. That is what makes the trial checkable by anyone. Nine screening questions, then the consent acknowledgments, your typed name, and the date. Sending it does not enroll you: the study screens every application by its published rubric, and you get one of two emails back — your enrollment confirmation with your code, or a screened-out note. Every acknowledgment is conditional: if enrolled, you consent; if you are screened out, you are not a participant and none of them binds you. Read the consent form, the Terms, the duty map, and the privacy note first — they are the four pages linked beside the acknowledgment below. Emails during the trial will not receive a response. If you do not complete the trial within your window, you will be logged as incomplete.

The name you enrol with is held privately; the report prints the author name on your Zenodo record.
If yes: that project can’t be the trial project — apply with a different one.
The consent acknowledgments — check every one (each applies if you are enrolled) Read them here: Consent Form · Terms · Duty Map · Privacy Note — each opens in a new tab.

2. Intake

Use this after your enrollment confirmation and after you have downloaded the kit version named in your enrollment confirmation, but before your first recorded session.

Copy the published version-and-fingerprint line. This is data, not a narrative answer.

3. The exit form

Your last step, on the Reader page. It asks for your code; your DOI goes on it; and before you send, the page shows you exactly how you will appear in the report and asks you to make sure your name and your DOI are correct. When you press Submit, the exit form makes two lines from your answers. Your public line holds everything that prints — it goes in the report’s appendix word for word, and it is the only thing in the QR image. Your private line holds the seven report-private answers; the study keeps it, counts it into totals, and never prints it. Both lines go in the same exit email. Your private line is held by the study and never printed; it is not encrypted, and email is a postcard.

4. Name and DOI correction

Use this only after the study emails you that your DOI check failed, for one of four reasons: your DOI did not open · you gave the “all versions” DOI instead of the version DOI · the record does not hold your Earned Trust deposit · the author name on the record does not match the name on your exit line. Before you resend, open your Zenodo record and copy the version DOI and the author name exactly as they appear there. A correction is accepted until your window closes, or 14 days after the correction email, whichever is later.

The author name exactly as your Zenodo record shows it — this is the name the report prints.
Copy the DOI from your Zenodo page — don’t type it. It must be the version DOI (the one shown on the record page for that version, not the “all versions” DOI).

5. Withdraw

No reason is asked. If you already sent your exit form, it stays in the study's data and cannot be pulled back.

The Field Kit — your ETR AI protocol inside

Field Kit v0.9.19 · SHA-256 88ed4c7b759c7933ad7d803c25090ec4e5ab9eb46a391a408dae223484a43f2c. Your enrollment confirmation names the version your trial runs on and links its file; download that one after your confirmation, copy its version line into your intake form, and do not begin a recorded session before then. If this page shows a newer version during your trial, ignore it.

The five checks, and who runs them

After your exit is in, the principal investigator (PI) runs five public checks from your deposit alone — your DOI opens; your files download; your QR card scans and opens the index in the Reader; your seal manifest is there and complete; and every fingerprint matches. A named second reader, Patrick Alejos, M.Ed., re-runs all five on a quarter of the deposits. Both sets of results publish.
